Question 1: What is my policy and ID #? Answer: Your policy number is 3W72 and your ID number is the number on your card. If you lose your card you can call Carefirst and Identify your self by using your social security number but your social security number is not your ID number for filing claims. Question 2: When does my coverage begin? Answer: Coverage begins the first of the month following the date you become a full-time employee, not the date you start working for ConTemporary. Example: If you start working for ConTemporary on 4/1 as a part-time employee and then become a full time employee on 7/25, your insurance will begin on 8/1. Question 3: When do I need to decide if I would like to enroll myself and/or my dependents? Answer: We need to receive your enrollment form within 30 days of your eligibility date. Question 4: How long can I keep my dependent children on the plan? Answer: Unmarried children can remain on the plan until the end of the month following their 23rd birthday. Unmarried children who are full-time students can remain on the plan until they graduate...age is not a limiting factor. Question 5: What should I do if I lose my cards? Answer: Don't panic! You can still get medical attention by giving your doctor and pharmacist your group # and ID # or SS#. In addition you can have them call Blue Cross to verify coverage. Lastly, contact the HR department so we can order replacements. Question 6: What happens if I need to fill a prescription before I get my prescription card or my pharmacist cannot verify my coverage? Answer: You will still have coverage; however, you will need to pay for the prescription up front. Call Care First immediately and order new cards, once received you can return to the pharmacy and ask to have your prescription re-processed. You will receive a refund minus the appropriate co-pay. Please keep in mind that you need to do this within 14 days otherwise you will have to fill out and file the prescription claim form. Question 7: What do I do if I have an emergency? Answer: If there is time, call the 800 # on the back of your card to find an in-network provider or hospital near you. If there is no time, go to the nearest hospital or provider and get the treatment you need, then call Blue Cross ASAP to get the proper authorization. Question 8: Do I have to pick a primary care physician to get the in network benefits? Answer: If you choose the HMO or the HMO -HRA alternative plan you will need too select a primary care physician and go through the referral process to see a specialist. If you choose the PPO plan you do not need to select a primary care physician nor do you need to obtain a referral to see a specialist. |

Your CareFirst Insurance Card will be sent via mail to the address you provided on your enrollment form. While you wait for your card to arrive, you are covered under our plan. To avoid paying out of pocket and then submitting claims, bring your temporary ID card to any doctors appointments you have before you receive your card.
Follow the steps below to print ID cards for you, the subscriber, and/or your dependents. When you create an account you will be asked to enter your Member ID (since you do not know your member ID yet, enter your Social Security Number), date of birth, username, password, and email address.
Please note, you will NOT be able to print your temporary ID card prior to your effective date of coverage.
1. create an online CareFirst account
2. Log in using your username and password
3. Click on the 5th option listed "Request an ID Card"
4. Select "Print Verification of Coverage"
5. Select the member you would like to print a verification card for (yourself or your dependent)
6. You can then save and print your verification of coverage/ID card
CareFirst's Online services allows you to view status of your claims, view the claims of your dependents, send secure emails to customer service, print Temporary ID cards, and more.

Go to www.jhancockpensions.com where you can register and enroll. Call the CTS payroll department for the CTS contract number and enrollment number. Print out the signature sheet and fax it to the Payroll fax 866-767-7297. Or, if you prefer to enroll by paper, call the Payroll office to receive a paper enrollment packet.
On the John Hancock website there are tools to help you calculate how much you will need for retirement, ways to allocate your contribution, and information on an array of personal financial topics. Once enrolled, you can manage your investment allocation easily.
